This came out of a 1993 Fred Meyers Challenge Event Book/Program - which I purchased as it had this one and many other players autographs in it. See my other listed items for many of the other players.
You will receive the original Letter of Authenticity by JSA as shown in the pictures.... I was certainly going to keep this myself as it's honestly a bucket list item for me but figured I would list it to see what might happen and if someone else might be interested in it for a good price. Thanks!
